FORMS AND FILING

The first
part of the Program will provide a step by step guide to preparing the paperwork
for an Uncontested Divorce, including the following: Motion to Proceed as a
Poor Person, Summons, Complaint, Affidavit of Service, Affidavits of Plaintiff
and Defendant, Sworn Statement of Removal of Barriers to Remarriage,
Affidavit/Affirmation of Regularity, Findings of Fact and Conclusions of Law,
Judgment of Divorce, Note of Issue, Part 130 Certification, Uncontested
Matrimonial Request for Judicial Intervention, NYS Certificate of Dissolution
of Marriage and Notice of Entry. The
second part of the Program will involve a mock interview demonstrating best
practices for obtaining the information needed to prepare the pleadings.
